The NSW Government has provided the official forms for Fire Safety Certificates and Annual and Supplementary Fire Safety Statements. To help you understand how to fill out the new forms, we have included a sample copy of a completed Fire Safety Statement. This sample can guide you in choosing a Competent Fire Safety Practitioner, as required by the Department of Planning and Environment. The building owner or their representative must fill out this form.
